Do you take insurance?

No. Elevate does not bill insurance. Largely because the majority of commercial insurance payers clsssify hormone replacement therapy as "elective" and therefore, do not cover the treatment regardless of how it is provided. Elevate is a cash-pay clinic. That being said, Elevate will complete medical necessity forms for the patient to submit for reimbursement.
